There are many different kinds of lawsuits and there isn’t a form for each type. If you want to file a lawsuit for money only and for up to $5,000, you can file in Small Claims Court and use the forms provided on the District Court page. For other kinds of lawsuits, the law library may have samples that will be useful but you would need to create your own legal documents.
